AAMI Surgical Gown Levels Explained: A Buyer's Guide

What Are AAMI Surgical Gown Levels?

When procuring surgical gowns for your facility, understanding AAMI (Association for the Advancement of Medical Instrumentation) protection levels is essential. The AAMI PB70 standard classifies surgical gowns into four levels based on their barrier protection against liquid strike-through and microbial penetration.

Choosing the wrong level can compromise patient and staff safety — or result in unnecessary cost. This guide breaks down each level so your procurement team can make confident, compliant purchasing decisions.

AAMI Level 1 — Minimal Risk

Level 1 gowns offer the lowest barrier protection and are suitable for basic care, standard isolation, and cover gown use. They are tested for resistance to water spray only.

Typical use cases: General ward care, visitor gowns, low-risk procedures with minimal fluid exposure.

AAMI Level 2 — Low Risk

Level 2 gowns provide protection against small amounts of fluid penetration. They are tested for resistance to water spray and hydrostatic pressure.

Typical use cases: Blood draw, suturing, pathology labs, ICU care.

AAMI Level 3 — Moderate Risk

Level 3 gowns offer moderate barrier protection and are tested against more rigorous hydrostatic pressure. They are appropriate for procedures with a moderate risk of fluid exposure.

Typical use cases: Arterial blood draw, IV insertion, trauma cases, emergency room procedures.

AAMI Level 4 — Highest Risk

Level 4 gowns provide the highest level of barrier protection and must pass a viral penetration resistance test. They are required for procedures with a high risk of contamination from blood-borne pathogens.

Typical use cases: Long, fluid-intensive surgical procedures, infectious disease cases, procedures involving drug-resistant organisms.

Key Considerations When Purchasing Surgical Gowns

  • Match the level to the procedure risk — over-specifying adds cost; under-specifying creates risk.
  • Check for CE marking — CE-marked gowns confirm compliance with EU Medical Device Regulation (MDR) standards, important for facilities operating under European regulatory frameworks.
  • Single-use vs. reusable — single-use gowns eliminate reprocessing costs and cross-contamination risk; reusable gowns may be more sustainable for high-volume facilities.
  • Verify supplier certifications — always request documentation of AAMI PB70 test results from your supplier.
